Humana offers three types of Medicare Advantage plans that include all the benefits of Original Medicare and many plans include prescription drug coverage.

• HumanaChoice (PPO) plans can give you freedom to choose and ways to save. Except for emergencies and urgent care, covered services cost less when you go to providers in the network.
• Humana Gold Plus (HMO) plans offer broad coverage with affordable, fixed costs. Care is coordinated through a network primary care physician, or PCP. You must receive all routine care from plan providers.
• Humana Gold Choice (PFFS) plans deliver flexible coverage that’s still affordable. You’re free to choose any doctor who accepts Medicare. With the exception of emergencies and urgent care, it may cost more for covered services received outside the network.

A Humana Medicare advantage plan is available to anyone enrolled in both Part A and Part B of Medicare. Medicare beneficiaries may enroll in the plan only during specific times of the year.
